My appointment time is reserved just for me and a late cancellation or missed appointment leaves a hole in my practitioners day that could have been offered to another patient in need of care; as such 24 hours notice is required for any cancellation

If I miss a scheduled appointment of provide less than 24 hours notice of my cancellation, I will be charged a cancellation fee in the amount of my contribution to the appointment had I appeared and received the scheduled treatment

In order to receive care at Wellness Hub Okato, the clinic and associated health practitioners are authorised to collect my personal and medical information and communicate with my family doctor and/or other health professionals involved in my care as deemed necessary for my benefit

Any information will be collected, held and protected according to the current privacy act; my medical information is confidential and will only be disclosed to third parties with my permission

FOR ACC TREATMENT

I authorise my records to be collected or disclosed to ACC to help determine cover for my claim and entitlements, or for ACC's research purposes (for injury prevention, or assessment and rehabilitation)

In order to receive care at Wellness Hub Okato:

 

You acknowledge that the medical and health information you have provided is complete and correct to your knowledge, you authorise treatment by a qualified practitioner to provide service as necessary for your illness, injury or condition and to help reach and maintain your health goals, your practitioner will explain every procedure and product to you and give ample opportunity to have questions answered prior to each treatment or procedure, you will be given opportunity, upon request, to read or be fully advised of clinic and treatment information including any rare but possible adverse events related to a service or product, you have the absolute right to decline part or all treatment being recommended/provided and this right extends throughout the treatment itself, it has been explained that touch/palpation is an integral part of an acupuncture/massage treatment session and that it may be necessary to remove some of my clothing the enable treatment to proceed; any area not actively being treated will be covered, you are at all times able to have a support person present in the room during treatment, and especially if any area of your breasts or genitals need to be exposed in order to receive appropriate treatment, you have the absolute right to a second opinion before treatment, you are liable to pay for 1) any private treatment, or surcharge in the case of an ACC treatment; 2) cancellation fees per the cancellation policy; 3) Any treatment that ACC declines to subsidise; 4) cost of medicinals and materials (discussed in advance).
